The crash occurred just after 1:30 p.m.
UPDATE 3:40 P.M. All lanes were cleared at 3:20 p.m. after a vehicle just after 1:30 shut down all lanes of the outer loop of I-495 S near mile marker 44, just before the Dulles Toll Road/VA-267 (exit 45). Back-ups and delays stretched three miles at the height of the shut-down. Were you caught in the mess on 495? Incident that shut down HOV lane remains under investigation
A four-vehicle crash Tuesday afternoon on I-66 West remains under investigation by the Virginia State Police. Spokeswoman Deborah Cox said two of the drivers involved in the crash were charged with reckless driving and following too closely, respectively. According to the department's preliminary investigation, the crash was a result of a chain reaction that began when a Dodge Caravan, traveling between Gallows Road and the Nutley Street exit, struck a Honda motorcycle. The impact pushed the motorcycle into a Subaru Outback, Cox said, which struck a Ford van. All of the vehicles were traveling in the left HOV lane. Helicopters, K-9 units responded to apprehend driver
The driver of a car that hit a stopped vehicle on Chain Bridge Road late Tuesday night was arrested and charged with driving while intoxicated and hit-and-run. William J. James, 42, of Sterling, was taken to the Fairfax County Adult Detention Center last night and released this morning on personal recognizance, a representative from the detention center said. An arraignment is scheduled for 8:30 a.m. Thursday. Fairfax County Police Department spokesman Don Gotthardt said at about 10:30 p.m. Tuesday night, a 2005 Jeep Liberty was stopped in the right lane of Route 123 Southbound at Oak Valley Drive when a 2010 BMW, allegedly driven by James, hit it from behind. A 56-year-old man died in a crash near Franklin Farms on Tuesday

A Isuzu Trooper crashed into a Thoreau Middle School classroom late Monday afternoon. A Fairfax County Police Department dispatch officer said a 911 cell phone call was placed to the department at 5:46 p.m. A non-load-bearing wall and a window at the school was damaged. The driver of the vehicle, whose identity has not been released by police, was assessed for injuries at the scene, but was not transported to the hospital. The dispatch officer said initial reports did not reveal the cause of the accident.
